Application Privacy Notice

This notice explains how Biogenyx handles the information you provide when you apply to the Biogenyx Referral Partner Program. It applies to this application page only.

We collect the information you enter in the application form, including:

• Your name, email, mobile phone number, city and state
• Your business or professional information, role, and business location
• Optional website and Instagram / social profile
• Approximate number of repeat clients you interact with in a typical month
• How you would naturally share the Biogenyx referral link or QR code
• How often relevant conversations come up and which topics are most relevant
• Why you are interested in the program and how you heard about it
• Required acknowledgements and optional communication consent

We also record basic technical context with your submission: the page you applied from, the referring link, campaign parameters in the URL, and a limited security signal used only to prevent automated or abusive submissions.

This is a business application form. Do not include patient names, health details or any medical information about yourself or anyone else. Medical questions are handled directly by the Biogenyx clinical team, never through this form.

We use your information to review and qualify your application, communicate with you about your application and the program, and administer the program if you are accepted. We also use it to measure recruiting and source effectiveness, prevent abuse and protect security, and meet legitimate legal and compliance needs.

Personal data submitted through this application process is not sold as part of this application process.

Applications are stored in a secure database. Access is restricted to authorized Biogenyx program administration. No online or database system can be guaranteed fully secure.

Necessary providers for hosting, database, security and email delivery may process data on Biogenyx's behalf.

We retain applications only as long as reasonably necessary to review and administer the program and to meet legitimate legal, security and operational obligations.

You can ask us to access, correct or delete the information you submitted by emailing Derek@biogenyxlabs.com.

Biogenyx uses reasonable technical and organizational safeguards to protect application data. However, no online or database system can be guaranteed 100% secure.
